What is a reproductive
endocrinologist?
Frequently, patients do not understand the difference
between a reproductive endocrinologist and a gynecologist.
The American Medical Association defines a reproductive
endocrinologist as "a specialist in obstetrics and
gynecology who is capable of managing complex problems
relating to reproductive endocrinology and infertility."
Both of these types of physicians have completed 4 years
of medical school, and 4 years of residency training in
obstetrics and gynecology. In addition, a reproductive
endocrinologist spends two to three additional years of
postdoctoral training specifically studying disorders
of female hormones along with the causes and treatment
of infertility.
When choosing a doctor for their infertility testing
and treatment, infertile couples need to understand
that an orderly and timely assessment of their fertility
factors will uncover the cause of their problem within
4 to 8 weeks. Rational treatment based on understanding
the nature of the infertility provides the best hope
of resolving the problem without wasting precious time.
It is best that a physician with knowledge and experience
in these matters manage your work-up. General gynecologists
are very good at the initial infertility evaluation
and can guide you through the early stages of your work-up
and treatment. If your doctor is a general gynecologist,
you should ask at what point he/she would refer you
to an infertility subspecialist (reproductive endocrinologist).
Often times, this should occur after about 6-12 months
of unsuccessful treatment. It will also vary depending
on the cause of the infertility - some cases should
be referred to a specialist immediately, such as when
the woman has an egg quality issue. Criteria for possible
referral to a reproductive endocrinology and infertility
specialist include, among others, female age over 37
years, blocked fallopian tubes or significant pelvic
scarring at any age, advanced degrees of endometriosis
at any age, significant male factor, and several months
of clomiphene citrate treatment without conception.
